A program is a specific grouping to which a patient is assigned. A program can be diagnostic, payer related, or referencing various care models. A patient might participate in an ACO program, hypertension program, or a Medicare Advantage program. Patients can belong to multiple programs depending on their conditions or payer assignment. Bamboo Health will show you the programs that the patient is attributed to at the point of referral using the pre-admit look-up tool or as you admit or discharge a patient. To view the list of programs a patient belongs to, and corresponding care instructions:

The pre-admit lookup tool allows you to view your patient's care team information upon referral. This is a simple way to determine a patient’s program prior to admission. Once you admit a patient attributed to a care program, a Ping will be sent to the other members of the patient’s care team.
