Programs: Overview

A program is a specific grouping to which a patient is assigned. A program can be diagnostic, payer related, or referencing various care models. A patient might participate in an ACO program, hypertension program, or a Medicare Advantage program. Patients can belong to multiple programs depending on their conditions or payer assignment. Bamboo Health will show you the programs that the patient is attributed to at the point of referral using the pre-admit look-up tool or as you admit or discharge a patient.

To view the list of programs a patient belongs to, and corresponding care instructions:

  1. Go to your patient roster tab and search for the patient's name.
  2. Click on their patient card which will bring you to their patient profile.
  3. Scroll to the care team and care instructions section of the patient's profile.

 Pre- Admit

The pre-admit lookup tool allows you to view your patient's care team information upon referral. This is a simple way to determine a patient’s program prior to admission. Once you admit a patient attributed to a care program, a Ping will be sent to the other members of the patient’s care team.

 Filter Top-Priority Patients by Programs

Using your saved filters in Bamboo Health, you can identify your admitted patients who are attributed to an ACO or other care management program.

Follow the steps outlined below to create a saved filter that identifies patients attributed to a specific ACO or care management:

In the filter pane on the left of your dashboard, select admitted under the current status header. Scroll down to the program header, and select your priority ACO or care management program.

Click create on the bottom of the panel.

Then, enter the name of your filter and click save!

Your new filter will now be listed under my saved filters! This filter will show only your admitted patients who are attributed to the ACO/care program that you selected.
