As a member of the Bamboo Health post-acute community, you can implement our platform in a variety of ways. For those post-acute organizations that do not directly integrate their EHRs with Bamboo Health we provide you with a number of options for indicating your patients' events while in your care. It’s critical that you update patient events in Bamboo Health as part of your existing admission and discharge process. This ensures that all members of your patients' care teams have the most up-to-date information at all times.

We've detailed how to take advantage of each of these actions below.

Admit a patient:

  1. Click admit patient in the navigation bar on your dashboard
  2. Enter the patient’s first and last name and click search.
    1. This will look through any previous records you've entered into Bamboo Health for a match. If the patient has been entered into Bamboo Health at your facility or organization before, you'll be able to build off their existing record instead of creating a new one.
  3. If a match is found, choose the patient record and click select. If a match is not found, click new patient.
  4. Complete the patient’s demographic details as needed, enter their encounter details, and click add admit.

You will receive a pop-up message if the patient matches to an Accountable Care Organization (ACO), physician organization, or any other population health management program in the PatientPing community. This pop-up will detail the patient’s program name (e.g. ACO), primary care provider, care coordinator contact information, and any care instructions tied to the patient's care plan. As soon as you see this pop-up window, a Ping will be sent to the patient's care management team as well, informing them of the patient’s admission!

Once you've completed the admission process, you can click into the patient's card from your currently admitted filter to the left in order to view the patient's full profile, including their prior encounter history, care team information, and care summary.

Note: It is a Bamboo Health best practice to enter all admissions and discharges onto your Bamboo Health dashboard. This is for two primary reasons:
  • You will be able to view your admitted patients' prior visit history details for events across the Bamboo Health community, regardless of whether those patients are attributed to an ACO or other care management program.
  • If your facility or organization is receiving Pings, or opts in to receive Pings in the future, those notifications are dependent on your recently admitted and discharged patient censuses. In order to take full advantage of the Pings service, it thus is critical that all patient events are entered into Bamboo Health. 

Edit an existing patient:

Once you've admitted a patient to your dashboard, you can edit any of their details as needed. This is especially helpful if you make an error or need to update an admission date, for example. To edit patient information:

  1. Click the edit pencil on the patient's card
  2. Select edit from the dropdown menu
  3. In the edit modal, modify the patient’s demographic information and/or details about their current event
  4. Click save after you have made your changes, or cancel to undo any changes


Indicate a payer change:

  1. Click the edit pencil on your patient's card.
  2. Select update status
  3. Choose payer change in the status dropdown
  4. Click switch insurance or add new insurance if your patient's updated payer information isn't listed
  5. Enter the date of the payer change, as well as the patient's updated payer details
  6. Click update

The updated payer information will be reflected on the patient card, in the patient's demographic information, as well as encounter history.

Discharge a patient from your facility or organization:

  1. Click the edit pencil on the patient's card
  2. Select update status
  3. Choose discharge in the status dropdown
  4. Enter the patient's discharge date and discharge disposition
  5. Click update

You're done! You’ve successfully discharged a patient. You will receive a pop-up message if the patient matches to an Accountable Care Organization (ACO), physician organization, or any other population health management program in the Bamboo Health community. This pop-up will detail the patient’s program name (e.g. ACO), primary care provider, care coordinator contact information, and any care instructions tied to the patient's care plan. As soon as you see this pop-up window, a Ping will be sent to the patient's care management team as well, informing them of the patient’s discharge!

Indicate an expired patient (same steps as above):

  1. Click the edit pencil on the patient's card
  2. Select update status
  3. Choose deceased in the status dropdown
  4. Enter the date on which the patient expired
  5. Click update

You will receive a pop-up message if the patient matches to an Accountable Care Organization (ACO), physician organization, or any other population health management program in the Bamboo Health community. This pop-up will detail the patient’s program name (e.g. ACO), primary care provider, care coordinator contact information, and any care instructions tied to the patient's care plan. As soon as you see this pop-up window, a Ping will be sent to the patient's care management team as well, informing them of the patient’s passing.

Transfer a patient:

  1. Click the edit pencil on the patient's card
  2. Select update status
  3. Choose transfer in the status dropdown
  4. Enter the transfer date and setting
  5. Indicate a related payer change, as applicable, by clicking add new insurance or switch insurance
  6. Click update

Transferred patients will remain on your currently admitted filter and their visit duration will reflect the number of days in their current setting. Your patient's broader care team also will receive a Ping on their transfer event.


Indicate a patient's Medical LOA:

  1. Click the edit pencil on the patient's card
  2. Select update status
  3. Choose medical leave of absence in the status dropdown
  4. Enter the date on which the MLOA began
  5. Indicate a related payer change, as applicable, by clicking add new insurance or switch insurance
  6. Click update

The patient’s MLOA status will be reflected up on their patient card, in their patient profile, and in exports. Patients on MLOA will remain on your currently admitted filter and their visit duration will reflect the number of days in their current setting.


You can indicate the end of a patient's MLOA with one of two statuses: resumed services or ended services.

  1. Click the edit pencil on the patient's card
  2. Select update status
  3. Select resumed services or ended services in the status dropdown
  4. Enter the date on which the MLOA ended
  5. Indicate a related payer change, as applicable, by clicking add new insurance or switch insurance
  6. Click update

The patient’s MLOA status will be reflected up on their patient card, in their profile, and in exports. Patients on MLOA will remain on your currently admitted filter and their visit duration will reflect the number of days in their current setting. 


As a new user, do I need to back-enter any data?

As a new user, you do not need to back-enter any data. Simply enter any new admissions and discharges from the date you start using Bamboo Health and be sure to enter those admissions and discharges in as close to realtime as possible.

A Bamboo Health best practice is to  enter all new admissions and discharges into Bamboo Health within 24 hours. This will ensure you're able to access valuable information from your patient's profile as soon as they begin receiving care from your organization. It also ensures that Pings on your patients' events are generated out to their broader care teams in as close to real time as possible. Timely data entry helps everyone provide more seamless and coordinated care.


How do I automate my data entry?
