Overview
Filters are additive; using multiple filter criteria will allow you to get as narrow, or as broad, a patient list as you need. When filtering within a single filter category, OR logic applies to your filter results. For example, if you select both admitted and discharged in the current status filter category, your results will show all patients whose most recent status is admitted OR discharged. However, when filtering across filter categories, AND logic applies to your filter results. For example, if you want to see all patients who are currently in the ED and are at a specific hospital, select presented under the current status filter category, and select your facility of interest under the facility name filter category. Your results will show only patients whose current status is presented AND whose current facility name is your facility of interest. Tip! Less is more when it comes to filtering. Users regularly encounter issues when they select too many filter criteria. When creating your saved filters, try starting with the filters under current status, facility name, facility type, setting, Bamboo Health flags, and program. Identify Missed Care Opportunities |
Missed care opportunities occur when a patient recently discharged from your facility returns to SNF care at another facility. Recognition of these events enables learning opportunities for potential workflow improvements to increase patient retention at your facilities. Follow the steps outlined below to create a saved filter that identifies missed care opportunities:
Identify Patients at Risk of Readmission |
The readmission risk flag applies to any patient currently in the ED or under observation who also has had a prior inpatient stay within the last 30 days. This flag identifies opportunities to reduce potential readmission penalties. Filtering your dashboard to patients with the readmission risk flag informs you of patients who have returned to the acute setting and allows you to notify their active care team of their recent/active services with your organization, should they be in need of additional post-acute care. Follow the steps outlined below to create a saved filter that identifies patients at risk of readmission:
Identify Patients Attributed to an ACO or Care Management Program |
Using your saved filters in PatientPing, you can identify your admitted patients who are attributed to an ACO or other care management program. Follow the steps outlined below to create a saved filter that identifies patients attributed to a specific ACO or care management program:
Identifying 3DW-Eligible Patients |
Managing patients eligible for the three-day rule waiver (3DW) supports the admission of a patient to a SNF directly from the ED or Observation, when clinically appropriate, without the requisite 3-day inpatient stay. Identifying these patients can support both improved patient outcomes and decreased total cost of care. Many of our ACO partners have provided Bamboo Health with the list of those facilities participating in their approved 3DW networks. If you are a member of such a network and your ACO is using Bamboo, you can indicate when patients are admitted to your facility under the 3DW. You will be able to do so only for patients who match to the ACOs with whom your facility has a 3DW relationship. This will provide your ACO partner(s) with equal transparency on those patients admitted to your facility under the waiver. It will also allow you to report specifically on your 3DW patient events through the Bamboo export feature. Creating saved filters by LOS in Bamboo expedites efforts to focus on patients by their LOS within your facility. Taking steps to reduce LOS can help decrease overall cost of care for patients in risk-based contracts, and improve patient outcomes and satisfaction. Follow the steps outlined below to create this saved filter to identify patients by their length of stay:
